The Youth Pathway Services Case Manager provides case management and coordinated support to young people participating in the Youth Pathways program. The role provides services within the wider youth focused Bright Futures Portfolio. Reporting to the Youth Pathway Team Leader, the role provides structured case management, coordinates support services, and works collaboratively with stakeholders to assist young people in achieving their goals and maintaining stability within the community. The role works with young people experiencing complex social challenges, including justice involvement, trauma, mental health concerns and housing instability. Using trauma-informed, culturally responsive, and strengths-based approaches, the Case Manager supports young people in developing life skills, building resilience, and working toward positive reintegration outcomes. What you’ll bring Relevant qualification in Youth Work, Community Services, Social Work or a related discipline (or equivalent experience). Demonstrated experience delivering case management or client support services within community services, youth services or justice-related programs. Strong understanding of trauma-transformative and strengths-based practice approaches. Experience supporting individuals with complex needs, including justice involvement, mental health challenges or social disadvantage. Strong interpersonal and communication skills, with the ability to build effective relationships with clients and stakeholders. Ability to coordinate services and manage multiple client cases effectively. Experience maintaining accurate documentation and client records. Ability to work in dynamic and challenging environments while maintaining professional boundaries Why join Outcare?
