Purpose of Position To deliver high-quality, patient-centred nursing care with a strong focus on chronic disease management, preventative health, care planning, and immunisation services, ensuring optimal patient outcomes and the smooth operation of the practice. Hours: Monday, Tuesday, Thursday, Friday, 8:30am-4:30pm - consistent hours with little to no variation. RESPONSIBILITIES Clinical Deliver evidence-based nursing care within scope of practice, with a strong emphasis on: Chronic disease management (e.g. diabetes, cardiovascular disease, respiratory conditions) Health assessments (including age-based and risk-based assessments) GP Management Plans (GPMPs) and Team Care Arrangements (TCAs) Immunisations and vaccination programs in line with the National Immunisation Schedule Perform clinical procedures including triage, wound care, ECGs, pathology collection, and assisting with procedures Provide patient education to support self-management of chronic conditions Maintain accurate and compliant clinical documentation Chronic Disease & Preventative Care Coordinate and deliver structured chronic disease management programs, including recalls, follow-ups, and monitoring outcomes Develop, implement, and review care plans (GPMPs and TCAs) in collaboration with GPs and allied health providers Conduct preventative health screenings and assessments, identifying early risk factors and initiating timely interventions Support patients in navigating services and accessing appropriate multidisciplinary care Vaccinations & Immunisation Programs Administer childhood, adult, and travel vaccinations in accordance with national guidelines Maintain vaccine cold chain compliance and accurate immunisation records Participate in recall and reminder systems to improve vaccination uptake Provide education to patients and families regarding vaccine safety and schedules Patient Services Assist with triage and coordination of care, particularly for patients with complex and chronic health needs Liaise with external providers to support integrated care Communicate results and follow-up care as directed by medical staff Advocate for patients and support engagement in ongoing care Quality Improvement & Health Outcomes Contribute to improving patient outcomes through: Preventative health initiatives Chronic disease registers and audits Recall and reminder systems for care plans, health assessments, and vaccinations Participate in community health promotion and patient education programs Equipment and Supplies Maintain clinical equipment, including vaccine storage and cold chain management Monitor and manage stock levels, ensuring availability of immunisation and clinical supplies Compliance Ensure compliance with all relevant legislation and standards including: Infection control and sterilisation Vaccine storage and handling requirements Accreditation standards and clinical governance Maintain up-to-date knowledge of Medicare item numbers relevant to care plans, health assessments, and immunisations Knowledge of RACGP standards of accreditations Professional Practice within nursing scope, code of ethics, and evidence-based guidelines Maintain current knowledge in: Chronic disease management Preventative health Immunisation practices Support education and knowledge sharing within the practice OHS Adhere to workplace health and safety standards at all times General Support and mentor other staff where appropriate Assist with general practice operations and administrative tasks as required Expected Personal Attributes Strong commitment to patient-centred, preventative, and chronic disease-focused care Excellent communication skills High attention to detail, particularly in care planning, documentation, and immunisation records Ability to work both independently and collaboratively within a multidisciplinary team Commitment to ongoing professional development EDUCATION, QUALIFICATIONS and EXPERIENCE ESSENTIAL Current nursing registration and relevant qualifications Minimum 5 years' experience, preferably in general practice or community health Demonstrated experience in: Chronic disease management and care planning (GPMPs, TCAs) Health assessments and preventative care Immunisation delivery and vaccine schedules Knowledge of Medicare item numbers related to chronic disease and preventative health Current CPR and emergency response skills Experience using clinical software (Medical Director Clinical, Pracsoft) Experience working with diverse populations Desirable: Additional qualifications in: Diabetes education Asthma/Spirometry Women's health APNA membership
