In the newly created role of Clinical Care Partner, you will be providing relationship-centred care management through a clinical lens that delivers exceptional client outcomes, strengthens wellbeing, and supports clients to thrive and live safely at home Day to day this role looks like: Achieving billable hours and service delivering expectations through effective care planning, funding utilisation, billing accuracy, and proactive management of client portfolio Undertaking person-centred care planning in accordance with Catholic Healthcare’s model of care Educating clients and their families on program guidelines, inclusions and exclusions and possible financial contributions Communicating and coordinating with members of the clinical, allied health and operational teams to identify client care needs, plan interventions, and review outcomes of care Completing clinical assessments (initial, annual and change in condition), using relevant assessment tools and evidence-based frameworks Delivering initial direct nursing care within RN scope of practice where practical in conjunction with assessment, prior to handover to ongoing nursing care to field nursing team Using screening, assessment and risk dashboards to identify high risk clients and prioritize timely reviews Initiating case conferencing to support hospital, rehab or respite discharge planning for wellbeing interventions, service adjustments and timely referrals Communicating and coordinating with members of the clinical, allied health and operational teams to identify client care needs, plan interventions, and review outcomes of care Promoting Catholic Healthcare Value proposition and service offerings while enabling clients to live at home independently with choice, control and connecting them to services that are safe, personal and effective Ensuring accurate billing, timely documentation and alignment of services with client goals and funding availability Understanding and adhering to the Aged Care Standards in all interactions, practices and documentation To be successful in this role you’ll need: Completion of tertiary qualifications in nursing, healthcare, or similar (Bachelor of Nursing) Current Registered Nurse registration with AHPRA Thorough understanding of the Aged Care Legislation and Aged Care Quality Standards Strong clinical knowledge and understanding of the ageing process, particularly age-related health issues Case management experience including assessment, care planning, goal writing and reviewing Previous experience working within a community setting in a similar role as a Registered or Enrolled Nurse Ability to work independently and as part of a multi-disciplinary team Excellent digital literacy, including Microsoft Outlook and Excel What we can offer you: Competitive hourly pay rate Km reimbursement (99c per Km) paid travel time Not-for-profit tax benefits so you pay less tax Fitness Passport discounted gym benefits Paid Parental leave Long Service leave after 5 years A values-based organisation that welcomes applicants from all faiths and backgrounds We know you have many choices of employer - choose Catholic Healthcare where you can make a positive impact and be well rewarded.
