Identify barriers, build care pathways and coordinate packages that improve flow and consumer outcomes. • Identify barriers and plan early discharge pathways • Coordinate hospital avoidance and discharge packages • Work with inpatient teams and community providers • Educate staff on alternatives to hospital care • Maintain activity data and support service improve About You You’re an experienced mental health nurse ready to support complex discharge and hospital avoidance.
