You’ll coordinate complex care, build strong partnerships and support teams to deliver safe, consistent and informed care across settings. Duties Coordinate complex viral hepatitis care and case management Deliver expert clinical care Lead care plans for clients with complex needs Educate and provide treatment to clients diagnosed with Hepatitis C Liaise with GPs, NGOs, prison health and partner services Provide outreach services Drive quality care and consistent practice About You You’re an experienced, compassionate nurse leader with strong HIV or infectious diseases expertise, ready to guide care, support teams and improve outcomes for diverse communities.
