About You : Certificate III or IV in Individual Support, Disability, Community Services or equivalent Position Summary : Prohealth Solutions (Mobility and Cares) supplies mobility equipment, clinical support and home-care solutions that support safer, independent living for people with disability, older Australians and NDIS participants. The organisation also assists clients to access equipment and services that support their health, independence and community participation. About the Role The Support Worker will deliver high-quality, person-centred support to clients in their homes and community settings. This role focuses on assisting participants to maximise independence, achieve personal goals and maintain safety while accessing mobility, care and daily living supports. Key Requirements : Essential Experience working with people with disability, older adults or vulnerable community members. Strong communication, empathy and interpersonal skills. Ability to work independently and as part of a team. Desirable Experience with mobility equipment or allied health support settings. Job Responsibility : Personal and Daily Living Support Support clients with tasks they find difficult due to disability or health issues, including but not limited to: Assistance with personal care (e.g. showering, dressing, grooming). Assistance with daily living tasks such as meal preparation, light household tasks (cleaning, laundry) and errands. Mobility and Equipment Support Assist clients with the safe use of mobility equipment (e.g. wheelchairs, scooters) supplied or recommended by Prohealth Solutions. Support safe transfers and positioning in accordance with clinician guidance and client care plans. Report equipment issues or service needs to the clinical or service coordination team. Community Participation and Transport Provide transport support or accompany clients to appointments, social and community activities as outlined in their support plan. Encourage and support clients to engage in community participation aligned with their goals. Emotional and Social Support Build trusting, respectful relationships with clients. Provide companionship and emotional support while encouraging independence and confidence. Documentation and Reporting Maintain accurate records of support provided, incidents, observations and any changes in client needs. Communicate relevant information to the service coordinator or manager in a timely manner. Compliance and Safety Follow all relevant organisational policies, procedures, and the NDIS Code of Conduct and Practice Standards. Ensure client privacy, dignity and respect at all times.
