About the Position An opportunity is available for a suitably qualified and experienced Community Worker to join the organisation on a full-time basis. This is a professional community services position responsible for the assessment of community needs, coordination of services, development of community-based programs and stakeholder engagement. The role contributes to the design and delivery of initiatives that improve access to services and enhance social inclusion for individuals and families within the community. The position is not a personal care or disability support worker role and does not involve the provision of daily living assistance. Key Responsibilities · Conduct structured assessments to identify community needs and service gaps across areas such as housing, employment, education, health and social inclusion · Design, develop and implement community-based programs and initiatives to address identified needs · Coordinate access to relevant community, government and private services through structured referral pathways · Maintain and strengthen referral networks with community organisations, government agencies and service providers · Represent the organisation at community forums, inter-agency meetings and stakeholder consultations · Promote awareness of community programs and services through community engagement and outreach activities · Collect and analyse data to monitor program outcomes and inform service planning · Prepare reports, submissions and program documentation as required · Provide structured guidance to individuals and families to support access to services and participation in community programs Selection Criteria · Relevant qualification in Community Services, Social Work, Welfare or a related field at AQF Diploma level or higher · Demonstrated experience in community services, program coordination, stakeholder engagement or community development · Knowledge of community service systems, referral pathways and local service networks · Strong written and verbal communication skills, including report writing and documentation · Ability to build effective relationships with individuals, families, community organisations and government stakeholders · Well-developed organisational, coordination and problem-solving skills Ability to work independently and as part of a multidisciplinary team Employment Conditions · Permanent full-time position · Standard business hours · Location: Brisbane region · Salary: $76,515 – $80,000 per annum plus superannuation Working with Hope & Care Community Services The organisation offers a supportive and inclusive work environment, with a strong focus on professional development, teamwork and the delivery of high-quality community services.
