About your opportunity: In this position you will Provide effective, efficient, competent, and culturally appropriate client focused clinical care to CHSP clients as per current Commonwealth Home Support Programme (CHSP) Manual 2024-2025. Promote cooperation, teamwork and understanding in undertaking generalist and/or specialist nursing practices for effective health outcomes. Effectively communicate at all levels across the multi-disciplinary, multi service team to promote effective team functioning and maintain accurate, timely documentation.
