As our services expand, we are seeking a compassionate, proactive, and motivated Community Worker to join our team full-time. Tasks and Responsibilities: Plan and coordinate community-based programs and services to support social, emotional, and practical needs of clients. Engage with individuals, families, and communities to assess needs and develop tailored support plans. Provide referrals and connect clients with appropriate health, welfare, housing, and employment services. Facilitate community education programs and awareness campaigns around health, wellbeing, and inclusion. Advocate on behalf of individuals and community groups to ensure access to essential services and supports. Collaborate with service providers, government agencies, and non-profit organisations to enhance service delivery. Monitor, review, and document client progress and outcomes, adjusting support strategies as needed. Promote community participation, inclusion, and social cohesion through outreach and engagement activities. Maintain accurate and confidential records in accordance with organisational and legislative requirements. Qualifications and Skills Certificate IV or higher in Community Services, Social Work, or related discipline. Minimum 1-2 years' experience in a community services or support role. Strong interpersonal and communication skills with an ability to build trust and rapport with clients from diverse backgrounds. Sound understanding of social and community issues affecting vulnerable populations. High level of organisation, initiative, and problem-solving skills. Ability to work independently and as part of a multidisciplinary team. Current Working With Children Check and Police Check (or willingness to obtain). Valid driver's licence preferred Salary This is for a full-time role with an annual salary of $78000 plus superannuation
