About your opportunity: In this position you will In particular, the social workers in Sub-acute services play a key role in discharge planning, leading team decision making and orchestrating the processes needed to arrange access to support services, NDIS and residential aged care. Utilise effective communication skills to form partnerships with internal and external stakeholders which enhance patient care and service delivery. Provide clinical practice supervision and mentoring of HP3 social workers, students and clinical support staff to ensure the maintenance of professional clinical standards within the social work services and multidisciplinary teams in the local facilities.
