Description The opportunity: We are looking for a Project Design and Delivery Lead to join our supportive and professional multidisciplinary teams on a fixed term contract until the end of June 2026. Responsibilities include but are not limited to; Lead redesign and delivery of needs-based, engaging, outcomes-focused social support for older adults. Collaborate with service design to integrate co-design and human-centred principles. Partner with marketing and local networks to promote the service and grow client engagement. Embed health and wellbeing outcomes with clear pathways to allied health, reablement, and HCP supports. Position the service as a soft entry into broader internal programs and care options. Oversee implementation and change management, supporting teams to adopt the new model. Provide expertise in ageing, wellbeing, and social connection informed by lived experience. Help older adults navigate and use their HCPs through tailored service mapping. Co-create personalised care pathways with healthy ageing, allied health, and design teams. Work with stakeholders to test, refine, and embed the service model. Use feedback and data to measure impact and drive continuous improvement. Skills And Experiences Our ideal candidate: Tertiary qualifications in aged care, community services, public health, social work, human services, or a related field. Demonstrated expertise in healthy ageing, wellbeing, community-based services, or social support. Extensive experience (5 years) in program or service design and delivery in ageing, wellbeing, or community-based settings. Demonstrated understanding of the aged care system in Australia, particularly Home Care Packages, and how to leverage them to support social and health outcomes. Experience working with or within care coordination, social support settings. Familiarity with human-centred design approaches and how to translate insights into action (you won’t be the designer — but you’ll work closely with them). Strong program and project management capability — able to balance vision with practical delivery. Excellent stakeholder engagement and communication skills, with the ability to build trust across teams and communities.
