Your new role The primary purpose of the Youth Outreach Case Manager position is to deliver effective outreach and holistic case management support for at-risk and vulnerable young people who are homeless or at risk of homelessness across the Casey, Dandenong, Frankston, and Mornington Peninsula local government areas. This role operates within an integrated, place-based service model in the Southern Region, contributing to improved outcomes for young people within their own communities. The Youth Outreach Case Manager focuses on facilitating access and navigation to services and support initiatives, promoting early intervention and prevention strategies, and establishing connections to broader health, welfare, education and employment service systems. Central to this role is the delivery of the Southern Homeless Youth Assistance Program (SHYAP), ensuring that all young people receive equitable opportunities to reach their potential within the strategic framework of the program. About you Demonstrated experience delivering outcomes-based services that consistently achieve contract performance targets. Experience delivering multi-faceted human services programs, preferably in the youth sector. Strong knowledge of case management principles and practice. Excellent stakeholder management skills, with the ability to form strong partnerships to achieve shared goals. Sound understanding of youth work practice, particularly Child Safety principles, and demonstrated ability to identify and manage risk. Understanding of the impact of homelessness on young people and families through direct experience or relevant work. Commitment to social justice, valuing the strengths, resilience, and courage of all children and young people. Proven teamwork and collaboration skills, with a proactive approach to supporting and promoting the success of others. Demonstrated cultural competence and the ability to work with Aboriginal and Torres Strait Islander peoples in a culturally safe manner. Motivated to drive performance and actively contribute to the growth of the organisation.
