About the role The Community Rehabilitation Program provides a short-term goal based interdisciplinary rehabilitation program for clients to optimize their functional abilities and independence in the home or community setting. The program operates out of 4 Eastern Health sites (Angliss Hospital, Peter James Centre, Wantirna Health and Yarra Ranges Health). We are seeking a client-cantered rehabilitation physiotherapist, with experience working in the community setting. This is an excellent opportunity to progress your career in a supportive environment, with regular supervision with a grade 3 physiotherapist and professional development sessions within the physiotherapy and multidisciplinary team. About you To be successful in this role you should possess: • Recognised tertiary qualification in Physiotherapy • Current registration with Physiotherapy Board of Australia • Eligibility for practicing membership of the Australian Physiotherapy Association • Exhibits positive professional behaviour at all times within the clinical and work setting. • Demonstrates a sound understanding and commitment to client centred care • Advanced clinical skills in managing a complex community rehab caseload and comprehensive discharge planning using a client-centred approach. • Broad clinical experience encompassing diverse patient presentation. • High level communication skills, both written and oral. • Demonstrated capacity to work / show leadership in a multidisciplinary team. • Experience in supervision of staff and/or students. • Demonstrated ability, knowledge and experience to lead the professional development of others • Demonstrated ability to introduce innovative practice. • Commitment to improving own practice and undertaking quality and research activities • Excellent time management skills • Ability to efficiently manage and prioritise a complex clinical workload. Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
