$41-$45 per hour super 4 days per week (Tuesday to Friday) with the ability to take on additional hours Opportunity for growth and development to management Our client is a not-for-profit organisation committed to supporting families and individuals with disability through Supported Independent Living options, various weekend activities and camp programs. Due to growth, we’re seeking a Support Coordinator to join their team in the Newcastle region to manage the needs of participants and execute the NDIS plans. The role: Coordinate care and support services for participants in line with funding and NDIS plan requirements. Create an environment that encourages participant engagement, enjoyment and participation based on meaningful and evidenced based activities and programs. Demonstrating the highest level of collaborative communication across all those involved to support an integrated approach to care delivery. Providing support, education and resources to consumers so they can make informed decisions and have choice and control over the services and support they receive. Conducting comprehensive assessments and developing goal oriented support and care plans in consultation with consumers and/or significant others in collaboration with health care professionals. Facilitating a pro-active case management process to promote consumer engagement and self-determination to achieve optimum quality of life within their own home. Managing individual consumer clinical and/or therapy plans within your own professional scope of practice or via appropriate delegation, referral and agreed reporting framework. Ability to attend Expos/events to encourage participant growth. What we’re looking for: Minimum Certificate IV and relevant experience in aged care, health or disability; or tertiary qualifications in relevant health discipline Ability to manage a case load of approx. 40 NDIS and aged care clients Good understanding of case management Excellent communication and interpersonal skills Demonstrated ability to work as part of a team and promote a positive workplace culture Ability to budget and negotiate with consumers around funding and spending arrangements. Ability to conduct comprehensive assessments and develop goal orientated care plans or willingness to learn Unrestricted Drivers Licence Sound knowledge and practical experience working with people who are ageing from an enablement, restorative and wellbeing perspective Experience working with people who have dementia and their carers Experience and commitment to working with quality and risk management systems Capacity to provide education and hands on learning in the workplace Experience working as part of a multidisciplinary team Benefits: $41-$45 per hour super 4 days per week (Tuesday to Friday) with the ability to take on additional hours Opportunity for growth and development to management If you’re ready to make a leap in your career with an amazing organisation, APPLY TODAY!
