At My Help Services, the position is responsible for assisting individuals with daily living activities, facilitating access to community resources, and promoting wellbeing through compassionate, client‑centred care. By working closely with clients, families, and support networks, the Community Worker ensures that services are delivered in line with NDIS standards, organisational values, and compliance requirements, directly contributing to the company’s mission of empowering people to live confidently and independently. Key responsibilities Community Engagement: Facilitate access to social, recreational, and community services to enhance inclusion and wellbeing. Care Planning: Work with clients, families, and support coordinators to implement personalised care plans in line with NDIS requirements. Advocacy: Support clients in expressing their needs and rights, ensuring their voices are heard in service delivery. Transport & Access: Provide safe transport and assistance to appointments, community programs, and essential services. Respite Care: Deliver short‑term relief services to families and carers, ensuring continuity of client support. Complex Care Assistance: Support clients with higher needs, including medication prompts, mobility support, and behavioural assistance. Record Keeping: Maintain accurate documentation of services delivered, incidents, and client progress for compliance and audit readiness. Health & Safety: Ensure safe working practices, adhering to workplace health, safety, and infection control standards. Collaboration: Work closely with multidisciplinary teams, allied health professionals, and external agencies to coordinate holistic care. Cultural Sensitivity: Provide services that respect cultural, linguistic, and individual diversity. Qualifications and skills Education: Certificate III or IV in Community Services, Individual Support, Disability, or Aged Care. Diploma or higher qualification in Social Work, Community Services, or related field is desirable. Experience: Minimum 1 years experience in aged care, disability support, or community services. Demonstrated ability to deliver person‑centred care and support in line with NDIS standards. Technical & Compliance Skills: Knowledge of NDIS framework, aged care standards, and community service compliance requirements. Proficiency in maintaining accurate service records and documentation for audit readiness. Understanding of workplace health and safety, infection control, and safeguarding practices. Other Requirements: Current First Aid and CPR certification. Valid driver’s licence and willingness to provide transport support.
