About the Role As a Case Manager, you play a vital role in overseeing your own caseload of clients and their support teams. This full time role is based in our Southport Head Office, don’t worry you do get out and about in the community visiting clients, onboarding new participants, training support teams and attending multidisciplinary meetings. You will: Manage a caseload of clients with a strong client-centred focus Oversee and review master rosters prior to rollover (we will train you!) Ensure staff are trained and supported to deliver high-quality care Liaise with allied health professionals and multidisciplinary teams Advocate for clients and support them toward achieving their goals Provide leadership and support to your client support teams We don’t do billable KPIs. Client satisfaction is our measure of success! Who We’re Looking For You might be: · A Registered Nurse (RN), an Enrolled Nurse (EN), an Allied Health Professional (Social Work, Psychology, Rehab, Human Services etc.) Or you could be an experienced Case Manager or Support Coordinator. To Be Successful You Will Have: Degree, Diploma or Advanced Diploma in Nursing, Community/Human Services, Social Work, Psychology, Allied Health or similar Previous case management and/or supervisory experience Support Coordination experience (highly desirable) Strong relationship management and advocacy skills Excellent written and verbal communication High attention to detail and critical thinking ability Strong IT skills Current Australian Driver’s Licence Blue Card and NDIS Worker Screening (or willingness to obtain) Availability to participate in paid on-call roster (approx. every 8–10 weeks once settled) Company vehicles are available for community visits.
