We are seeking applicants that are located in Hamilton and surrounding areas including Casterton What You'll Be Doing As part of our committed support at home team, you’ll provide safe, respectful, and person-centred care across: Person-Centred Care — Deliver compassionate, respectful, and individualised support that promotes client independence, dignity, and overall wellbeing within the home environment. Daily Living Support — Assist clients with general household tasks, shopping, and other activities of daily living to enable them to remain safely and comfortably in their own homes. Monitoring & Escalation — Identify and respond to changes in clients’ physical or emotional wellbeing, escalating concerns appropriately to ensure timely support and care. Consumer Engagement — Build positive relationships with clients, families, and carers through open communication, empathy, and a supportive approach to care. Teamwork & Collaboration — Work cooperatively with colleagues, carers, and the broader care team, contributing to a respectful and supportive team environment. Quality & Safety — Follow organisational policies, procedures, and care standards while contributing to safe practices and continuous improvement in service delivery. What You Bring Strong communication and interpersonal skills A genuine passion for supporting Older Australians, with a caring and respectful approach that promotes dignity, independence, and wellbeing. Ability to work effectively within a multidisciplinary team Ability to build positive relationships with clients, carers, and social supports through empathy, patience, and clear communication.
