About MNDAWA The Motor Neurone Disease Association of Western Australia (MNDAWA) delivers person-centered support and specialist care to people affected by MND in Western Australia. About the Role As an NDIS specialist support coordinator, you will provide varying levels of high-quality coordination of NDIS support for people living with MND. Working alongside a multidisciplinary team, you will support people living with MND to access the NDIS, access specialised services and advocate for the needs of people living with MND to ensure they have everything they need. You’ll work closely with the team to: Support and promote the choices of people living with MND Deliver person-centred coordination of care services Implement service agreements and oversee the coordination of services delivered by community service providers Coordinate multidisciplinary team meetings as required Advocate for and upskill community service providers on the needs of people loving with MND Work closely with the NDIS planners within the NDIS Priority Pathway Maintain records to support effective support service provision, performance analysis and reporting Partake in education provision relating to supporting people living with MND within the NDIS Participate and contribute to the overall MNDAWA services This is a fast-paced and hands-on role that offers variety, purpose, and the opportunity to build strong relationships across the disability sector. About You You’re a compassionate person with a strong drive to support others. You have high communication skills who thrives in a fast-paced, purpose-driven environment. You are able to support your clients balance creativity with strong attention to detail, and you’re passionate about creating experiences that inspire generosity. You’ll bring: At least 2 years’ experience as a Support Coordinator Tertiary, health, disability or community care related qualifications Excellent interpersonal skills Proven written and verbal communication skills Demonstrated excellent time management skills Demonstrated ability to work within billable targets and KPI’s Demonstrated high level of problem-solving skills and ability to work with minimal supervision Strong understanding of the NDIS, NDIS Practice Standards and ability to working within a changing disability sector Ability to manage risk, prioritise tasks, and meet deadlines under pressure Excellent time management, discretion, and professionalism Ability to develop and maintain effective working relationships with health and community service providers A valid WA Driver’s Licence and current Police & WWC Checks and NDIS worker screening Bonus: · Experience supporting a person living with MND Why Join Us?
