About the Role The Primary Health Care Nurse conducts comprehensive assessment, planning, implementation and evaluation of care in accordance with best practice and clinical guidelines. Working in partnership with the multidisciplinary team, this role delivers person-centred, high quality, safe care to people in detention. Operating within a nurse-led clinic model with health promotion as a core component, this role suits clinicians with recent acute experience who are comfortable working at a different pace to hospitals. You will support detainees with complex, multifaceted health needs in a culturally diverse environment - work that is both clinically challenging and deeply meaningful. For Aboriginal and Torres Strait Islander nurses, the cross-cultural dimensions of supporting detainees from diverse backgrounds make this a particularly resonant opportunity. Key Duties Work collaboratively with the primary health care team to deliver a multidisciplinary, coordinated and integrated service. Undertake a wide range of clinical duties including immunisations, blood collection, wound dressings, health assessments, care planning, medication administration and health promotion. Provide proficient, evidence-based chronic disease management. Participate in case conferences as required; liaise with internal and external stakeholders. Contribute to policy and procedure development and continuous service improvement initiatives. Maintain compliance with all legislative, OHS and documentation obligations. Essential Attributes / Skills / Experience General unrestricted registration by AHPRA as a Registered Nurse (Division 1). Relevant postgraduate qualification desirable. Minimum five (5) years clinical nursing experience (non-Telehealth) in an operational healthcare setting. Knowledge of contemporary issues in drug and alcohol health service delivery and relevant Commonwealth and State legislation, policies, standards and frameworks. Ability to prioritise, work independently and as part of a multidisciplinary team. High level communication skills, critical thinking ability and adaptability to respond to a changing work environment. Proficient computer skills including Microsoft Office Suite.
