Position Profile: We are seeking a dynamic and enthusiastic individual to appoint to a Senior Social Worker position that is available with the Assessment and Treatment Team (ATT) with the Midland Adult Community Mental Health Service. In this position, the successful applicant will work as part of a multidisciplinary team to assess, plan, implement and provide support as per the care coordination model at an advanced level of practice in the Assessment and Treatment Team (ATT). Should you be successful, you will plan, implement, and evaluate consumer safety and quality of care in addition to working as part of the ATT multidisciplinary team to share the role of triage and on-duty clinician over a 7 day roster which will include a combination of morning and afternoon shifts. You will also supervised Social Work staff and support staff in the clinical area, and practice as a Senior Social Worker and ensure practice is in accordance with the Australian Association of Social Workers Practice Standards 2023 and Code of Ethics 2020. Other key duties include, but are not limited to; Provides an advanced level of care coordination to Adult Mental Health consumers and their carers/families inclusive of mental state assessments, risk assessments, care planning, counselling, psychoeducation, discharge planning and referral and brokering of other services and team members . Undertakes triaging of new referrals and acts as duty clinician as required. Provides Social Work screening, assessment, treatment/intervention and evaluation at an advanced level of practice to complex and general caseload/s. Supports the delivery of safe patient care and the consumers’ experience including identifying, facilitating and participating in continuous safety and quality improvement activities, and ensuring services and practices align with the requirements of the National Safety and Quality Health Service Standards and other recognised health standards.
