About the Role As a Disability Support Worker, you will play a vital role in supporting participants to achieve their personal goals, build independence, and participate fully in their community. This role includes personalised in-home and community-based supports designed to enhance daily living, employment, social, and recreational skills. You will: Support clients in daily tasks, employment, social, and recreational activities Facilitate community access and encourage participation in activities and outings Provide person-centred direct care based on individual service plans Monitor client wellbeing, safety, and progress Maintain accurate documentation and communication notes Collaborate with families, carers, case managers, and allied health professionals Follow all work health, safety, medication, and privacy guidelines Participate in regular team meetings, supervision, and training opportunities About You You are compassionate, reliable, and committed to making a difference. Qualifications and Experience Certificate III or IV in Disability, Aged Care, or Individual Support (preferred) At least one year of hands-on experience in personal care (preferred) Experience supporting children or individuals with mental health disabilities (preferred) NDIS Worker Screening Check Current Police Check Working with Children Check (preferred) First Aid & CPR Valid Driver’s Licence and reliable vehicle Attributes Excellent communication and interpersonal skills Strong teamwork and problem-solving abilities Dependable, organised, and able to work independently Commitment to person-centred care and participant empowerment Professional, confident, and compassionate approach to support Other Requirements Understanding of NDIS principles and relevant legislation Current First Aid and CPR certification Completion of NDIS Worker Orientation and supporting practice modules Police Check, NDIS Worker Screening Check, and Working with Children Check Up-to-date Covid-19 vaccination Current driver’s licence and fully insured vehicle Full Australian working rights Why Join Relight Your Care?
