About the role and where you will be working: You’ll manage your own portfolio of clients, coordinating tailored care services that truly make an impact. Acting as the key link between clients, families, and care teams, you’ll ensure services are seamless, responsive, and centred around each individual’s needs and goals. Your responsibilities will include but not limited to: Coordinating personalised home care services that support independence and wellbeing Building strong, trusting relationships with clients, families, and stakeholders Monitoring and adjusting care plans to meet changing needs Managing client budgets and ensuring funding is used effectively Collaborating with internal teams and external providers for integrated care Ensuring compliance, quality standards, and accurate documentation Contributing to continuous improvement and client satisfaction About you: Willingness to embrace the values of Southern Cross Care WA and rewarding industry and how it plays into the role You’re a compassionate and organised professional who thrives on delivering person-centred care. With experience in aged or home care, you’re confident managing competing priorities, building strong relationships, and navigating complex situations.
