About us Home Caring Croydon Park is a reputable and fast-growing provider of NDIS and Aged Care services in the western suburbs of Adelaide. We are currently seeking a passionate and experienced Case Manager or Registered Nurse to join our team and support a growing number of Vietnamese-speaking clients in the Croydon Park area. This role is ideal for someone who is community-minded, culturally sensitive, and committed to providing high-quality, person-centred care. You’ll be working with clients across both Aged Care and Disability sectors. Key Responsibilities: Conducting client assessments and developing person-centred care/support plans Coordinating services, monitoring outcomes, and reviewing care plans regularly Advocating for clients and ensuring their cultural, linguistic, and health needs are met Supporting clients to access appropriate services and resources Liaising with families, support workers, and health professionals Ensuring compliance with NDIS and Aged Care quality standards Providing clinical oversight (for RN-qualified applicants) What We’re Looking For: Qualification in Nursing (Registered Nurse) and/or relevant Case Management experience Fluency in Vietnamese and English Experience in aged care, disability, or community health settings Strong communication, problem-solving, and organisational skills Understanding of NDIS and/or Aged Care systems Empathy, cultural awareness, and a passion for helping others Current AHPRA registration (for RNs) Current First Aid & CPR, National Police Check, and NDIS Worker Screening SA Driver's Licence and reliable vehicle Full working rights in Australia and COVID-19 vaccination Why Join Us?
