Are you a skilled and dynamic Family Support Worker looking for a challenging and rewarding role? Eternal Disability Care is seeking 2 motivated Family Support Worker to join their team. Job Duties As a Family Support Worker, you will Assess client needs and develop, implement, and review tailored support, education, and intervention programs Conduct client interviews to identify social, emotional, financial, and behavioural challenges Monitor client progress, maintain accurate case notes, and prepare reports for internal and external stakeholders Provide practical support and guidance to families, children, and vulnerable individuals, including those with disabilities Refer clients to appropriate community services, agencies, and support networks Liaise with community groups, government bodies, and service providers to coordinate care and promote available resources Support young people in addressing social, emotional, and financial issues Contribute to community development initiatives, including assessing service needs and available resources Assist in preparing reports, submissions, and documentation for funding and program development Requirements AQF Associate Degree, Advanced Diploma or Diploma in a related field Minimum of 1 - 2 years of relevant experience and/or on-the-job training may be required Effective communicator to foster energy and commitment within the organization. Skilled at inspiring individuals and teams to achieve organizational goals, motivating them to excel and develop by providing feedback, coaching, training, and guidance for career growth Ability to work with diverse and vulnerable populations Experience liaising with community services and agencies
