This is a hands-on coordination role responsible for ensuring participant supports are delivered safely, consistently and in line with NDIS standards. Key Responsibilities: Coordinate SIL clusters and assigned community participants. Actively engage with Participants and respond to changes in service needs and communicate changes with support workers. Ensure support plans and documentation remain current. Balance participant support needs with workforce sustainability. Work closely with rostering team to ensure shifts align with participant needs. Coordinate diaries and manage scheduling commitments. Proficient in Microsoft Office Suite and data entry. Current driver's license and access to a personal vehicle. Calm under pressure in a 24/7 support environment.
