Summary: We are seeking a highly motivated and compassionate Case Worker to join our team. The Case Worker will be responsible for providing support and assistance to individuals and families in need. Responsibilities: Conduct assessments to determine the needs of clients and develop individualized service plans Provide counseling and support to clients and their families Connect clients with community resources and services Monitor and evaluate client progress and adjust service plans as needed Maintain accurate and up-to-date client records Collaborate with other professionals and agencies to provide comprehensive services to clients Participate in team meetings and trainings Qualifications: Bachelor's degree in social work, psychology, or a related field Experience working with individuals and families in crisis Knowledge of community resources and services Strong communication and interpersonal skills Ability to work independently and as part of a team Excellent organizational and time management skills Compassionate and non-judgmental approach to working with clients
