About the Role Catholic Homes is seeking an experienced Nurse Practitioner (NP) to provide clinical support and expertise to Residents and staff. This is a full-time role reporting directly to the Head of Residential Care. The Nurse Practitioner performs advanced physical assessments and management of Residents' health status which may include, but is not limited to, the direct referral of Residents to other health care professionals, prescribing medications, and ordering diagnostic investigations. Key responsibilities of the role include, but are not limited to: Conduct comprehensive, relevant, and holistic health assessments using diagnostic capability Collaborate with GPs, interdisciplinary teams, and pharmacists to complete health screening assessments, carry out medication regime reviews Develop effective medication management plans and request investigations or make referrals as required Diagnose and treat a specified range of acute/chronic health problems Identify opportunities for clinical systems and liaise with Facility Managers/Clinical Nurses to progress Contribute to day-to-day operations of the residence to ensure the efficient delivery of high standards of clinical care and services to Residents Identify staff clinical education and development needs to ensure a sufficiently skilled and competent workforce that meets the ongoing needs of Residents Provide fair and equitable leadership to all staff including clinical, care, and ancillary The successful candidate will have/be: Essential applicants are registered with AHPRA as a Nurse Practitioner Demonstrate a commitment to continuous quality improvement and contributes to the Continuous Improvement Program Experience working in Aged, Health, or Community care Ability to lead and work well within a team with strong leadership qualities Commitment to person-centered care Passion and dedication for working with the elderly Enthusiastic and positive demeanor Good communication and interpersonal skills The ability and commitment to uphold high standards of work Benefits Ability to Salary Package - meaning you pay less tax and have more take-home money to spend Employee Assistance Program (free confidential counselling service) Comprehensive induction program and ongoing professional development Ongoing support from the Head of Residential Care and Senior Care Team About Catholic Homes Caring for Western Australians for 60 years, Catholic Homes is a leading not-for-profit comprehensive aged care provider.
