We are seeking a passionate and motivated Registered Nurse to step into the dynamic role of Clinical Care Coordinator . Key responsibilities include: Conduct home assessments and develop, implement, and update individualised care plans. Provide specialist nursing advice, supervision, leadership, and mentoring to care staff and coordinators. Deliver clinical care including wound management, medication administration, and treatments. Monitor client health, report changes, and review care plans as needed. Coordinate continuity of care with hospitals, doctors, allied health, and community services. Assess physical, mental, and psychosocial needs using a multidisciplinary approach. Support NDIS and My Aged Care program compliance and client goal achievement. Maintain accurate documentation and meet all policy, funding, and administrative requirements. Build strong relationships with clients, families, and carers to promote a customer-focused culture. Adhere to organisational policies, WHS standards, and complete general administrative and ad hoc duties. The successful applicant will have: Current AHPRA Nursing Registration . Strong foundational clinical knowledge and skill sets. High level of organisational and time management skills. Exceptional professional verbal and written communication abilities. Demonstrated commitment to delivering high-quality customer service and maintaining positive client and stakeholder relationships Proficiency in computer skills, particularly in Microsoft Office and Excel.
