Our commitment lies in delivering in-home services 7 days a week for Support At Home recipients and 5 days a week for CHSP recipients. We prioritize trust and continuity by guaranteeing our clients consistent support workers and scheduled services, thereby upholding their well-being and dignity. Description The role: The Clinical Care Partner will bring a clinical focus to our person-centred care model, supporting elderly clients to achieve their goals and maintain independence. This role includes acting as a liaison with clients, families, caregivers, and health professionals while offering clinical insights and support to the team. Your responsibilities may include: Managing a case load of home care packages, complex, and high needs clients Ensuring services meet internal and external quality standards Driving innovation and continuous improvement aligned with ECHO’s strategic objectives Offering guidance and support to clients, families, and team members to deliver safe, effective care Hours: Monday to Friday, 8:00 am to 4:00 pm. You will also be required to participate in a rotating on-call after hours, weekday and weekend roster. Skills And Experiences We are looking for someone who brings experience, compassion, and dedication to community-focused care. In your application, please address the following criteria: Essential: Registered Nurse with a tertiary qualification in nursing APRAH registered Minimum 2 years’ experience in a case management role within aged care or community care Demonstrated experience in providing clinical assessments, wound care, medication administration, continence management, chronic disease management (including diabetes), palliative and dementia care.
