Based in Ballarat or across the Grampians Health region, you will work autonomously to prevent avoidable hospital admissions, support early discharge and strengthen clinical capability within residential aged care. You will partner with older people, Nurse Practitioners, GPs, Geriatricians, ward teams, aged care staff and Emergency Departments to deliver safe, coordinated care aligned with residents’ goals and the Strengthened Aged Care Quality Standards.
