To read more on what it's like to work for West Moreton Health: What it's like to work with us | West Moreton Health About this role The HITH team operates collaboratively with acute inpatient hospital services, community-based services, residential aged care facilities, and multidisciplinary Allied Health services to provide evidence-based care to patients who meet specific criteria. We also provides care in the community by reviewing patients with acute illness in their place of residence, who would otherwise be required to remain in hospital to receive the same care. About you Our ideal candidate will be someone who can demonstrate the following: Contemporary clinical knowledge in the acute nursing within the hospital and community settings, delivering competent nursing care in complex situations while providing support to other team members. Ability to comprehensively assess, plan, implement and evaluate nursing care. Demonstrated ability to provide patient and family centred care to the patient using best practice and within the standard of practice of a Registered Nurse. Demonstrated ability to communicate to a high standard effectively and engage with members of the multidisciplinary healthcare team, patients, relatives, and visitors whilst displaying a high level of customer service. Mandatory Requirements: Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16 Appointment to this position requires proof of qualification and current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
