What You'll Do: Deliver high-quality clinical nursing care in line with individual client needs and care plans Complete assessments, develop care plans, and provide wound care, catheter management, stoma care, diabetes management and chronic disease management Collaborate with GPs and allied health professionals to ensure holistic care Empower clients through education and promote health and wellbeing What We’re Looking For: Division One RN, registered with APHRA & minimum 1 year of experience in community, aged care, or acute settings Proficient use of technology, including mobile platforms and provision of telehealth services.
