The Care-Coordinator is responsible for the provision of effective service coordination working closely with clients with the aim to achieve improved health outcomes for all Aboriginal and Torres Strait Islander people. Duties Provide Care Coordination to assist in the provision of chronic disease management and promote health care and planning as per IUIH Model of Care. Ensure appropriate Management and Coordination of individual Chronic Disease Care Plans and Health Checks. Provide quality Aboriginal primary health care by assisting the primary health care team in effectively planning and coordinating the workload to achieve results. Promote and assist clients with complex care to a model of self-care. Review clients with collaborative case conferencing. Skills And Experiences Minimum Bachelor in Nursing Full un-restricted Registered Nurse AHPRA registration. Previous demonstrable experience in relevant nursing field. Demonstrated ability to meet targets and performance outcomes. Ability to support a continuous improvement model for achieving outcomes. Decisive listening, influencing and persuasive skills Preferred Minimum two (2) years’ experience in a primary health setting. Previous experience of working with Aboriginal and Torres Strait Islander clients. Previous experience in a multidisciplinary health team. Previous experience in chronic disease management.
