Position Title: Registered Nurse Care Coordinator – Complex Care (HARP) Classification: ZF4 Salary per hour: $59.72 superannuation generous salary packaging options Employment Type: Fixed Term Contract Part-time 48 hours per fortnight until 27th June 2026 Award: Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024 -2028 Role description As a Registered Nurse Care Coordinator within the HIP Complex Care Service (CCS), you will provide person-centred care coordination for clients with complex psycho-social and healthcare needs. This role focuses on supporting clients to improve their self-management skills, health, and wellbeing through a collaborative approach. You will: Conduct comprehensive holistic health assessments in clients' homes and local communities. Coordinate all aspects of health and social care needs, including referrals to specialist services. Monitor progress toward client goals and facilitate interventions from other health professionals. Ensure seamless linkages between hospital and community services. Deliver high-quality, evidence-based clinical interventions in line with professional standards and scope of practice. About you Registered Nurse with current AHPRA registration. Experience in chronic disease management, complex aged care, or community health. Strong assessment, care planning, and coordination skills. Ability to work autonomously and collaboratively within a multidisciplinary team. Excellent communication and problem-solving skills.
