About the role This role leads and coordinates care for patients with complex clinical needs who access outpatient services at the Royal Brisbane and Women's Hospital. You'll act as the central point for communication, providing specialist nursing leadership, education and evidence-based guidance to improve patient outcomes and care coordination across the health continuum. Key Responsibilities Provide advanced nursing leadership and specialist clinical advice to improve coordination and quality of outpatient care. Facilitate collaboration and communication between patients, carers, and multidisciplinary teams to support continuity of care. Lead quality improvement and service development initiatives to enhance care models and patient outcomes. Deliver education and mentoring to nursing and inter-professional staff to strengthen outpatient capability and model-of-care consistency.
