Role: As a Case Manager, you’ll be responsible for coordinating care and delivering trauma-informed, holistic support to children and young people. You’ll work collaboratively with carers, birth families, and service providers to ensure each child’s wellbeing, safety, and cultural connections are prioritised. You’ll join a supportive, passionate team that’s dedicated to creating positive outcomes for children and families. Key Responsibilities Manage a caseload of children and young people in OOHC, tailoring your approach to meet their individual needs. Conduct assessments and develop, implement, and review case plans in partnership with children, young people, carers, and families. Engage clients using appropriate risk assessment and intervention strategies. Maintain transparent communication and collaborate with relevant agencies and service providers. Coordinate wrap-around support to address the holistic needs of clients. Criteria: Tertiary qualifications in social work, early childhood, psychology or similar. Minimum of 12 months case management experience working with disadvantaged children, young people & families Experience undertaking comprehensive assessments A strong understanding of child protection issues and client vulnerabilities such as drug and alcohol use, domestic violence, mental health issues, and more.
