We're looking for a dynamic and organised Home Care Coordinator to oversee the day-to-day delivery of our Community and In-home Support programs, including Home Support Workers and Social Support services. As the Home Care Coordinator, you'll be responsible for ensuring a reliable and responsive customer experience through effective scheduling, coordination, and service recovery. You'll own the daily service plan, manage rostering and shift optimisation, and lead backfill and surge responses. Your work will directly support our mission to provide timely, compassionate, and consistent care to clients in their homes and communities. This role does not hold clinical governance. All clinical decisions and escalations remain under the Clinical Care Coordinator. About you Tertiary qualifications in a health, community services or social services related discipline or related experience Proven experience in service coordination, rostering, or operational leadership in community or aged care settings. Strong organisational and problem-solving skills. Excellent communication and collaboration abilities. Commitment to person-centred care and service excellence.
