Your new role: As an Aboriginal and Torres Strait Islander Health Worker in Cardiology, you will play a key role in improving access, equity, and health outcomes for Aboriginal and Torres Strait Islander patients. Working within the Heart, Lung and Critical Care division, you will deliver culturally appropriate support, education, and advocacy across cardiac care services. Key responsibilities include: Supporting patients, families, and carers during cardiac care journeys, including discharge and follow-up Providing health education, case management, and culturally safe advocacy Liaising with Aboriginal and Torres Strait Islander community groups and organisations Attending appointments with patients and linking them to community or primary health services Promoting and facilitating access to cardiac services and supporting patient self-management Contributing to quality improvement, governance, and community consultation initiatives This is an identified vacancy with a genuine occupational requirement for the position to be filled by a candidate indigenous to the Aboriginal or Torres Strait Islander Community. Candidates not meeting this criteria will not be considered.
