About your role As the QSCIS Transition Coordinator, you will provide advanced clinical expertise and leadership across the spinal rehabilitation continuum - from early intervention to discharge and community reintegration. You will: Apply advanced clinical reasoning and judgement in complex spinal injury rehabilitation and discharge planning Lead system-wide transition coordination initiatives that enhance patient flow and continuity of care Provide specialist consultation and advice to multidisciplinary teams, supporting safe and coordinated patient outcomes Mentor and guide less experienced clinicians and support workforce capability development Drive quality and service improvement activities, including research and evidence-based practice translation Engage with internal and external stakeholders to strengthen partnerships across hospital, community, and disability sectors About you You're an experienced clinician with a passion for improving patient outcomes and leading system change. You thrive in multidisciplinary environments and enjoy mentoring others while influencing service delivery at a strategic level.
