About your role: Assess the needs of Aboriginal and Torres Strait Islander patients and their families/carers and provide relevant culturally appropriate intervention, i.e. problem solving, practical assistance and referral. Provide comprehensive patient management within a multi-disciplinary team by providing information about hospital services, participation in discharge planning, attending family meetings, undertaking referrals to internal and external services. To participate in the Aboriginal and Torres Strait Islander Liaison Service activities such as in-service programmes, business plan activities, staff meetings, statistical collection/collation and record keeping.
