We are seeking passionate and dedicated Disability Support Workers to join our team and make a meaningful impact in the lives of the people we support. Position Overview: As a Disability Support Worker, you will play a crucial role in assisting individuals with disabilities to live independently and fully participate in their communities. You will provide personal care, support with daily activities, and ensure that each individual receives the highest level of care and assistance. Key Responsibilities: Support clients to engage in daily activities at home and in the community, including age-appropriate tasks, social outings, and employment-related activities. Assist clients with personal care, hygiene, dressing, mobility, and meal preparation according to individual care plans. Accompany clients to community activities, appointments, and outings, promoting social inclusion and participation. Provide companionship, friendship, and emotional support to enhance clients’ overall well-being. Carry out light household tasks as required, including cleaning, shopping, and running errands. Implement individualized care plans while adhering to policies, including medication administration, client rights, privacy, and workplace health and safety. Monitor the safety and well-being of clients and report any concerns to the manager. Maintain accurate documentation of care provided, including client notes, communication books, and progress reports. Participate in the planning, implementation, and evaluation of programs and activities to support client goals. Collaborate with families, carers, case managers, and multidisciplinary teams to provide consistent, client-focused support. Act as a key carer for designated clients, preparing monthly reports on progress. May live with the client where required to provide ongoing support and care. Contribute actively to a positive team environment through open communication, staff meetings, planning processes, and supporting colleagues as needed Key Requirements: Certificate III or IV in Individual Support (Disability) or equivalent. Previous 1-year experience in disability support or a related field is highly desirable. Understanding of the National Disability Insurance Scheme (NDIS) and its policies. Strong communication and interpersonal skills. Ability to work both independently and as part of a team. A caring, compassionate, and empathetic attitude towards individuals with disabilities. Current first aid and CPR certification (or willingness to obtain). Current Working with Children Check (WWCC) and National Police Check.
