About the role South West Healthcare (SWH) is seeking an experienced and passionate Healthy Communities Coordinator to lead our dynamic Health Promotion team. In this pivotal role, you will work collaboratively with key stakeholders, partner organisations, and the local community to design and deliver initiatives that improve health equity and support the wellbeing of people across South West Victoria. Reporting to the Primary and Community Services Directorate, you will oversee the planning, development, implementation, coordination and performance against a dynamic SWH Health Promotion Plan. This role will coordinate a range of projects to deliver health promotion initiatives across the continuum, by working closely with our partners, to ensure a fully integrated approach that builds on existing strengths, and leads to better health outcomes in the South West region.
