Case Management Officer – First Nations Identified Role Location: Brisbane, QLD Employment Type: Full-time, Permanent Salary: $81,000 15% Super Role Purpose Support and empower vulnerable, isolated, and at-risk individuals through a culturally responsive, client-centered, and strengths-based case management approach. This role is dedicated to First Nations people and aims to uphold self-determination, dignity, and holistic wellbeing for Aboriginal and Torres Strait Islander clients. Key Responsibilities Client Engagement & Support Deliver direct client services including intake, needs assessment, support planning, and coordinated referrals to community and legal services. Build trusting relationships with clients to support informed decision-making and goal achievement. Facilitate the development and review of Case Management Plans (CMPs) in partnership with clients. Case Management Manage an active caseload of up to 20 clients, each presenting with diverse and complex needs, vulnerabilities, and risk factors. Provide ongoing support, advocacy, and coordination of services tailored to individual client circumstances. Records & Data Management Accurately document all client interactions, daily case notes, and service activities using the Client Management System (CMS). Ensure all client records, consents, and authorities are current and securely maintained in line with privacy and confidentiality standards. Complete required forms, checklists, and aggregate CMP data for monthly statistical reporting and evaluation. Stakeholder Coordination Liaise with internal legal teams and external social support services to ensure seamless and culturally safe referrals. Regularly update regional stakeholder databases and maintain accurate contact records. Team Collaboration & Continuous Improvement Actively contribute to a supportive and collaborative team culture. Participate in service reviews, team meetings, and initiatives aimed at enhancing practice and outcomes for First Nations clients. Requirements Identified position: Applicants must identify as Aboriginal and/or Torres Strait Islander. Demonstrated understanding of the cultural, social, and systemic factors impacting First Nations communities. Experience in case management, social work, or community services (preferred). Strong interpersonal, documentation, and organizational skills. Commitment to confidentiality, ethical practice, and client empowerment. This position requires full-time, office-based attendance in the Brisbane CBD, five days per week. Remote or work-from-home arrangements are not available. A valid driver’s licence is essential, as occasional travel may be required as part of the role.
