The Client DATNM Health Care Services is a disability support and health care services company based in Somerville in Western Australia. They are looking for a full time Community Worker at Somerville WA. The Role This is a full-time position based in Somerville, WA, offering an annual salary of $75,000 to $85,000 plus superannuation, depending on experience. The role is based on a 38-hour week with a rotating roster, including weekend shifts and reasonable overtime as required. Duties will include: As part of the community support team, you will work closely with individuals to help improve their quality of life, independence, and connection with the wider community. The following duties reflect their day-to-day responsibilities: Follow individual care and support plans, and help deliver practical education, training, and personal development programs tailored to each client's goals Observe, monitor, and document clients' wellbeing and progress after each shift through accurate, up-to-date case notes Participate in care team meetings and contribute to assessments focused on the client's ongoing needs in areas such as health, housing, employment, and community access Provide one-on-one support to clients in their homes or supported accommodation, including adult disability services and shared living environments Help clients address personal, emotional, and financial challenges through active support, encouragement, and connection with relevant services Assist with preparing reports for government bodies or other agencies to support funding, program delivery, or compliance requirements Accompany and support clients during social outings, such as grocery shopping, appointments, community events, or skill-building activities Escort clients to medical appointments, therapy sessions, hospital visits, and respite services to ensure continuity of care and emotional support Maintain a clean and safe environment for clients when necessary, including light cleaning or helping with laundry or bedding, as part of their overall wellbeing and hygiene Requirements A Diploma in Community Services, Cert 3 or Cert 4, or a related qualification is preferred Minimum 2 years' experience in community or aged care settings Minimum 2 years of relevant experience in a professional setting Minimum 2 years of relevant experience in community or aged care Minimum 2 years of experience dealing with mental and emotional disturbances and developmental disability Minimum 2 years of experience working with government agencies, non-profits, and other service providers Strong communication and interpersonal skills, with a caring and professional approach Must hold or be willing to obtain a current First Aid (HLTAID011) and CPR (HLTAID009) certificate Must have a valid driver's licence and a clear National Police Check Must be eligible for or hold a current Working with Children Check Willing to work on a rotating roster, including weekends and reasonable overtime as required This full-time position is based in Armadale, WA, with a salary of $75,000 to $80,000 plus superannuation, depending on experience
