We provide education and support to family members and GP and can refer services tailored to individual needs. Your role at St John of God Social Outreach (DAWN) We are seeking applications for a Clinical Nurse Specialist (AOD) to join our Subiaco DAWN team on a permanent full-time basis. You will work collaboratively with GP’s and Medical Specialists in the community in order to provide a safe and supportive service to consumers. This will include providing specialist information and/or treatment, including clinical information and care for home-based withdrawal from alcohol and other drugs. Provide quality, culturally sensitive management and support to consumers and their families/significant others who are seeking specialist information and/or treatment, including home base withdrawal from alcohol and other drugs. The Position Provide triage when required, assessment, case management and specialised clinical interventions to consumers with substance use and or dual diagnose disorders, utilising evidence based practice. Provide education for consumers and their families/significant others regarding the withdrawal process. Demonstrate and maintain advanced clinical skills and knowledge in the area of alcohol and other drugs and or dual diagnosis to ensure the delivery of a high standard of consumer care. Where appropriate, report changes in consumer’s condition to Manager and/or medical practitioners and document in consumer Electronic Medical Record (EMR). Ensure appropriate risk management strategies are employed in the identification, assessment and investigation of risk issues. Implement appropriate strategies to control identified risks. Provide joint assessments and support at various locations (including consumer’s home) in a shared care arrangement with other specialty organizations, to provide quality care to consumers with complex substance use, medical and mental health needs. You will have relevant post registration experience in working with alcohol and drug using clients, registration with the Nursing & Midwifery Board of Australia as a Registered Nurse and well-developed skills in communication, conflict resolution, negotiation and problem solving. Successful applicants must hold a valid WA drivers licence, current CPR, current first aid and a Working with Children card.
