It aims to optimise independence, mobility, and overall wellbeing through comprehensive, person-centred care that addresses medical, functional, cognitive, and social factors, often involving a multidisciplinary team and tailored care planning. About the Role As an Enrolled Nurse within Rehabilitation and Geriatric Rehabilitation service, you will play an essential role in delivering safe, person ‑ centred care to clients with diverse and complex needs. You will: Assist in the delivery of goal-oriented rehabilitation programs aimed at restoring function, mobility, and independence Support the care of older adults with complex needs, including frailty, cognitive impairment, and chronic health conditions Undertake clinical duties within scope of practice, including monitoring vital signs, assisting with activities of daily living, wound care, and medication administration (as endorsed) Promote patient dignity, independence, and wellbeing through respectful and culturally safe care practices Assist with discharge planning by supporting patient education and coordinating with community services where appropriate Maintain accurate and up-to-date clinical documentation in accordance with organisational policies and legal requirements This is an opportunity to form ongoing, meaningful relationships with clients and families while being fully supported by a team renowned for its expertise, kindness, and collaborative culture. You will have: Current registration with the Australian Health Practitioner regulation Agency as an Enrolled Nurse (Division 2) with a Nursing and Midwifery Board of Australia approved qualification for administration of medication (including intravenous medication administration). Knowledge of rehabilitation nursing preferred but not essential and commitment to quality improvement, practice development and Evidence-based practice . Demonstrated effective clinical communication through the use of information technology and written, verbal and interpersonal skills Understanding of team nursing & demonstrated ability to work effectively as part of the multi-disciplinary health care team Applicants must have current Australian work rights.
