The team provides comprehensive assessment, intervention, and discharge planning across acute, subacute, and community settings, working collaboratively within a multidisciplinary framework. About your role: Utilise high level clinical knowledge, expertise and decision making based upon current evidence, to provide safe and efficient service delivery to multiple service areas on a daily basis across the continuum of care. Provide leadership within the interdisciplinary and multidisciplinary teams in promoting and conducting activities aimed at continuous quality improvement including the planning, development, delivery and evaluation of clinical services service, review of clinical practices, work instructions and procedures. Involvement in service provision to patients as part of the Occupational Therapy Department public holiday and weekend service, and out of hours roster as operationally required. Foster and maintain well developed links with internal (including multidisciplinary team) and external stakeholders to ensure coordination of patient care in a professional manner.
