Role Summary The Community Worker provides person-centred support, advocacy, and practical community-based assistance to individuals, families, older persons, and people living with disability experiencing social, emotional, health, financial, or personal challenges. The role involves supporting clients to maintain independence, wellbeing, dignity, and community participation through the delivery of in-home and community supports, monitoring client wellbeing, assisting with access to services, coordinating referrals, promoting safety and inclusion, and working collaboratively with clients, families, coordinators, providers, and allied health professionals in line with individual support plans and organisational policies and procedures. Key Duties and Responsibilities Provide day-to-day care and supervision to participants in accordance with individual care plans, organisational procedures and participants needs. Support participants with daily living activities, including personal care, hygiene, dressing, meal assistance and mobility support where required. Monitor participant wellbeing, behaviour and day-to-day functioning, and report concerns or changes to supervisors and relevant professionals. Maintain accurate records including progress notes, incidents, observations and care documentation. Support participant to develop independence, routine, participation and life skills within the residential setting. Assist with planned activities, structured programs and social engagement opportunities for participants as outlined in care plans. Maintain a safe environment and follow workplace health and safety, infection control and safeguarding procedures. Promote culturally appropriate, respectful and person-centred support practices. Contribute to community engagement initiatives, structured support activities and wellbeing programs. Provide bilingual support and culturally sensitive communication where required. Work cooperatively with team members, health professionals, families and other stakeholders involved Selection Criteria Relevant Diploma or higher qualification in Community Services, Welfare, Disability Support, Individual Support, Social Work or a related field. Minimum of at least one year of post-qualification experience in community services, disability support, aged care, welfare support or a related environment. Strong understanding of community support services, referral pathways and person-centred practices. Ability to maintain accurate documentation and client records. Strong communication, teamwork and client-support skills. Current First Aid/CPR Certificate and National Police Check. Current Working with Children Check or willingness to obtain. Current driver license and reliable transport preferred. Bilingual or multilingual communication skills highly desirable.
