General Information Position Title: Support Coordinator – Implementation Specialist Team: Implementing Team Reports to: Implementing Team Lead Location: Hybrid (Office/Community/Remote) Classification: SCHADS Award (Competitive) Core Responsibilities 1. Assessment & Strategic Planning Execute person-centered assessments to align participant goals with available NDIS funding. Develop individualised implementation plans that prioritise participant choice and control. Oversee the initial rollout of supports, ensuring all services are coordinated and meeting immediate needs. 2. Intensive Capacity Building Deliver tailored training to participants and their support networks on NDIS self-navigation. Empower participants to manage their own supports effectively, moving them toward independence. Identify and connect participants with high-quality, impartial service providers. 3. Complex Case Management & Resolution Identify and address immediate funding gaps or service delivery barriers. Manage complex stakeholder dynamics and escalate specialist issues to the Team Lead as required. Ensure proactive and holistic communication between participants, families, and providers. 4. Documentation & Compliance Maintain meticulous case notes and records within the CRM system. Ensure all practices adhere to the NDIS Quality and Safeguards Commission standards. Prepare comprehensive handover reports for the Monitoring Team once readiness criteria are met. Qualifications & Requirements Education: Tertiary qualification in Human Services, Social Work, Psychology, or a related discipline. Experience: A minimum of 3 years in the disability sector with a focus on complex case management. Compliance: Valid NDIS Worker Screening Check. Working with Children Check (WWCC). National Police Check. Transport: Full Australian Driver’s License and a reliable, insured vehicle.
