About the role As a Client Care Coordinator / Service Coordinator , you will help coordinate day-to-day services for participants and support workers across our Home Care operations. You will work closely with participants, families, support workers, internal teams and management to ensure services are delivered safely, consistently and in line with participant needs. You will be supported by the Senior Service Coordinator for day-to-day task direction, workflow support and escalation guidance. Your responsibilities will include: Coordinating roster changes and managing short-notice call-ins Arranging shift coverage within approved participant-matching and compatibility guidelines Communicating with support workers, participants, families and internal teams Responding to low to moderate incidents, service concerns and operational changes Ensuring incidents and concerns are documented and escalated appropriately Maintaining accurate participant and service records Assisting with participant support plans, risk assessments and service agreements Supporting continuity of care during busy and critical service periods Monitoring service delivery and identifying follow-up actions Working within SHCS policies, procedures and escalation pathways Supporting frontline workers with clear communication and practical guidance Hours of work This is a full-time office-based role located in Wollongong. Standard office hours: Monday to Friday, 9:00am to 5:00pm The role also includes a rotational after-hours on-call component , shared between Service Coordinators. On-call coverage includes: Weekdays: 5:00pm to 9:00am Saturdays and Sundays: full-day coverage when rostered On-call rosters will be planned and communicated in advance. On-call allowances apply when rostered. About you You may be a strong fit for this role if you: Have experience in disability, aged care, Home Care, community services or a similar fast-paced service environment Can manage competing priorities calmly and professionally Communicate clearly with workers, participants, families and internal teams Are confident making practical decisions within operational guidelines Understand the importance of documentation, compliance and escalation Enjoy problem solving and supporting frontline teams Can multitask while still maintaining attention to detail Are reliable, organised and able to follow through on tasks Want to grow into a senior coordination or leadership pathway Essential requirements Certificate III in Individual Support or equivalent qualification Minimum 2 years’ experience in disability, aged care, community services or a similar high-demand service environment NDIS Worker Screening Check Current First Aid and CPR Current driver’s licence and access to a reliable vehicle Strong computer and communication skills Why join SHCS?
